Adult SMI Case Manager — Community Care & Coordination

Jewish Family & Children's Service of Arizona is seeking an Adult SMI Case Manager to provide case management services to Seriously Mentally Ill individuals. The role involves coordinating with healthcare providers, maintaining communication with clinical teams, and performing community-based duties including transportation. Ideal candidates have experience in behavioral health, a High School Diploma (Bachelor's preferred), and must be able to acquire a Fingerprint Clearance Card.
