Responsibilities
- Provide one-on-one physical therapy in patients' home settings, delivering personalized plans of care to improve mobility and independence.
- Assess patients' condition, develop and adjust plans of care, and collaborate with an interdisciplinary team to meet identified needs.
- Demonstrate and document standard therapy measurements, including range of motion, strength, and balance testing.
- Utilize a variety of therapeutic modalities to maximize functional outcomes for clients.
- Case manage a home health caseload, incorporating OASIS-driven processes and EMR documentation.
- Communicate progress and recommendations with the care team and participate in plan-of-care development.

Requirements
- Active Vermont Physical Therapist license
- Home health experience and OASIS experience required
- McKesson or Netsmart EMR experience strongly preferred
- Ability to work in a home-based setting and manage a patient caseload
- Willingness to start by the assignment start date and travel as needed for home visits
