Two Harbours Healthcare Ltd Health Inequalities Nurse – Children & Young People Closing date: 31 May 2026 This is an exciting opportunity for a senior nurse or advanced clinical practitioner to work across Weymouth and Portland to support development of services for children and young people, with a particular focus on inequalities. The role is part‑time (16 hours per week) on a fixed‑term 12‑month contract, with the possibility of becoming permanent. Responsibilities Integral member of the PCN team and the local CYP Multi‑Disciplinary Team (MDT). Represent and champion the health needs of local children and young people. Contribute to the development of the role and new ways of working. Keep abreast of local and national data and policy pertaining to CYP and act on them. Work with CYP, carers, families and other professionals to survey and understand barriers to accessing healthcare and shape services accordingly. Create links and communication pathways with local relevant teams and services (child health immunisation team, health visitors, school nurses, schools, public health teams, youth services, GP practices, asylum‑seeker and refugee support teams, libraries, citizen advice, local family hubs and food banks). Develop and maintain innovative methods for GP practices to communicate with CYP. Record and monitor the activities and impact of the role and share this information with the PCN board on a quarterly basis. Ensure effective planning and allocation of time across all areas of work. Key Areas of Work PCN GP Practices – Lead a local network for GP practice CYP champions to support practices in providing CYP‑friendly services, including appointment systems, practice websites, consent/confidentiality information, and use of IT resources. Audit GP practice care provision for CYP on request and provide advice for improvement. Care Leavers Support – Work with social services care‑leavers support teams to support access to general practice services for young care leavers and ensure GP practices provide reasonable adjustments where required. Transition between Child and Adult Services – Provide information and links for young people and their carers, including those with learning disabilities, and support education on GP and other health services up to age 18. Education – Work with local schools, colleges and youth services to provide health information and education sessions, tailor sessions to meet confidentiality and consent requirements, and provide healthcare careers advice at local career fairs. Vaccinations – Deliver the UK childhood and adult vaccination programme, maintain practical skills, review local vaccination data to identify children with outstanding vaccinations, and support flexible provision (home visits, evening or weekend clinics). Qualifications Essential: Registered Nurse. Experience Experience of working with children and young people – essential. Experience of working in a Primary Care environment – desirable. Experience of working autonomously – desirable. Other Information Salary: £25.26 – £29.42 per hour (depending on experience). Contract: Fixed term, 12 months. Working pattern: Part‑time, flexible.
