The Cardiac Nurse Practitioner provides advanced nursing care to patients with cardiovascular conditions, supporting diagnosis, treatment, education, and ongoing management across the continuum of cardiac care.
Responsibilities
- Assess, diagnose, and manage patients with acute and chronic cardiovascular conditions.
- Obtain medical histories, perform physical examinations, and evaluate diagnostic results.
- Develop and implement individualized treatment plans in collaboration with cardiologists and other healthcare professionals.
- Prescribe medications and therapies within applicable scope of practice.
- Monitor patient progress and adjust care plans based on clinical response.
- Educate patients and families about cardiovascular conditions, treatments, medications, and lifestyle modifications.
- Document clinical findings, treatments, and outcomes accurately in the medical record.
- Coordinate follow-up care and referrals to support continuity of treatment.
This position is a temp contract position for 1.5 days per week. Must be able to work
