Physical Therapist - Outpatient Neuro Rehabilitation

Location: On-Site

Schedule: 24–32 Hours Per Week

Position Summary

We are seeking a compassionate and skilled Physical Therapist to join our interdisciplinary rehabilitation team. This position provides direct patient care in a specialized rehabilitation setting, helping individuals achieve their highest level of independence and functional recovery.

The Physical Therapist will provide treatment services to a diverse patient population, including individuals recovering from neurological injuries and conditions as well as orthopedic diagnoses. Experience treating patients with Traumatic Brain Injury (TBI), stroke, spinal cord injury, joint replacements, musculoskeletal injuries, post-surgical rehabilitation, and general orthopedic conditions is preferred.

Responsibilities

• Evaluate, develop, and implement individualized physical therapy treatment plans.
• Provide therapeutic interventions to improve mobility, strength, balance, endurance, and functional independence.
• Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and case managers to coordinate patient care.
• Document evaluations, treatment plans, progress notes, and discharge summaries in accordance with regulatory and organizational standards.
• Educate patients, families, and caregivers regarding treatment goals, home exercise programs, and safety recommendations.
• Participate in interdisciplinary team meetings and patient care planning.

Qualifications

• Current Physical Therapist license in the State of Michigan.
• Graduate of an accredited Physical Therapy program.
• Minimum one (1) year of clinical experience preferred.
• Neurorehabilitation experience preferred. Experience treating both neurological and orthopedic patient populations, including TBI, stroke, spinal cord injury, joint replacements, and post-surgical orthopedic cases, is highly desirable.
• Strong communication, documentation, and clinical reasoning skills.
• Current CPR certification or ability to obtain upon hire.
