Hospice RN Case Manager: Lead Interdisciplinary Care
Big Bend Hospice
The RN Case Manager – Care and Comfort in Tallahassee, FL develops and coordinates the interdisciplinary plan of care for hospice patients to ensure quality of life. The role includes symptom management, patient/family education, and timely assessments with documentation in EMR. Supervises Hospice Aide and LPN while collaborating with IDT and physicians. Minimum one year inpatient nursing or hospice/home health experience is required with a Florida RN license; travel within the area is expected #J-18808-Ljbffr Big Bend Hospice
