Hospice Case Manager Nurse — Home-Based Patient Care
Affinity Health Management
Affinity Health Management is seeking a Registered Nurse to plan, organize and direct hospice care in home and community settings. The nurse will use community resources to meet patient and family needs and will work within an interdisciplinary team. Responsibilities include comprehensive patient assessments, care planning, administering medications, caregiver education, and coordinating care as case manager.
