Responsibilities
- Administer prescribed medications and treatments per physician orders, documenting accurately in the resident’s chart.
- Monitor residents’ conditions, respond to changes, and escalate to the supervising RN as needed.
- Assist with activities of daily living and provide compassionate, patient-centered care to a geriatric population.
- Perform routine nursing procedures, collect data (vital signs, measurements), and participate in care planning and updates.
- Collaborate with the interdisciplinary team to implement care plans, ensure safety, and support infection control and standard precautions.
- Maintain precise documentation, report incidents, and contribute to a respectful, compliant care environment.

Requirements
- Active Vermont LPN license.
- Graduation from an accredited practical nursing program.
- Ability to work evenings and/or overnight shifts in a long-term care setting.
- Prior long-term care or geriatrics experience preferred.
