Shelter Social Worker: Case Management & Housing Support

BXFN

Bronx Family Network seeks a Social Worker to provide case management, counseling, and resource coordination for shelter clients. You will assess needs, develop service plans, and connect clients with housing and DHS-supported services. Responsibilities include conducting assessments, tracking progress, and coordinating with outreach, healthcare, education, and law enforcement to deliver comprehensive support.
