Nurse Practitioner/Physician Assistant

Nurse Practitioner/Physician Assistant Position Summary : Provide the full scope of integrated primary care services, which fall under his/her field of training, including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance to patients. As a member of the Christ Community Health (CCH) team, providers are expected to be familiar with the mission, vision and philosophy of ministry of CCH and work collaboratively within their scope. Key responsibilities include: Work closely with Behavioral Health Consultants (BHCs), and other care team members to provide holistic (medical, behavioral & spiritual) integrated care to CCH patients; Collecting, recording, and maintaining patients' information and histories; Diagnosing and treating illnesses, medical conditions, and injuries; Ordering, administering, and analyzing diagnostic tests; Prescribing and administering treatments, therapies, medications, vaccinations, and other specialized medical care; Training patients and their families to manage and prevent illness and injury.
