Work Environment

This position is primarily onsite, requiring you to travel to patients' homes within the Santa Ana area. Our home care model emphasizes flexibility and personalized attention, allowing you to build meaningful relationships with your patients while providing essential therapeutic services. You will work independently, scheduling patient visits, while also collaborating closely with our administrative team for seamless operations.

Key Responsibilities

• Conduct thorough assessments of patients' physical conditions, history, and personal goals to develop tailored rehabilitation plans.
• Implement evidence-based therapeutic interventions including therapeutic exercises, manual therapy, and specialized rehabilitation techniques.
• Educate patients and their families on their treatment protocols, progress timelines, and necessary home exercise routines to encourage active participation in recovery.
• Collaborate regularly with physicians, nursing staff, and other allied health professionals to enhance patient care and optimize treatment outcomes.
• Maintain accurate and detailed records of patient evaluations, progress notes, treatment plans, and outcomes in compliance with healthcare regulations.
• Stay current on the latest advancements in physical therapy practices and integrate new techniques into your therapy sessions to enhance patient care.
• Provide emotional support and motivation to patients throughout their recovery process, fostering a positive therapeutic relationship.

Patient Population / Client Focus

In this role, you will primarily work with patients recovering from orthopedic surgeries, sports injuries, and chronic pain disorders. Our patient population often includes individuals seeking rehabilitation following joint replacements, fractures, and soft tissue injuries. Your expertise will directly contribute to improving their quality of life, helping them regain function, and promoting overall wellness in a supportive home environment.

Qualifications

• A Bachelor's Degree in Physical Therapy is required; advanced qualifications such as a Master's Degree or Doctorate are preferred.
• A current and active state license to practice as a Physical Therapist in California.
• Current Basic Life Support (BCLS) certification through AHA is required upon hire and must be maintained.

Preferred Experience

• A minimum of 2 years of experience in an outpatient orthopedic setting is preferred.
• Strong skills in manual therapy and a background in treating sports injuries, post-operative rehab, or chronic pain management will enhance your candidacy.

Compensation and Benefits

We offer a competitive compensation package ranging from $110,000 to $125,000 per year for full-time employees, commensurate with experience.
