Nurse Case Manager (NCM), Army National Guard, Phoenix, AZ Work Location Arizona Army National Guard Office of the State Surgeon 5650 E McDowell Road Phoenix, Arizona 85008 Citizenship Requirements: Must be a U.S. Citizen. Clearance Requirements: National Background Check with Inquiries and Fingerprints, and Drug Screen. Type of Employment: Full-Time, 40 hours per week. Total Compensation: $$45.18 per hour Period of Performance: 01 September 2026 - 31 August 2031 Benefits: Health/Dental and Vision Paid Time Off per Year: Up to 80 hours per year, which starts accruing on the day of hire. Sick Leave per Year: Up to 56 hours per year, which starts accruing on the day of hire. Paid Federal Holidays: Eleven (10) Paid Federal Holidays Summary of Services Registered Nurse - Nurse Case Manager (NCM), a non-clinical nurse case manager, is to assist in reaching their goal of a full, medically/dental-ready Unit Manning Roster (UMR) and, upon alert, the Deployment Manning Document (DMD). They function as liaison between the unit of assignment and the Office of the State Surgeon. Working Hours Registered Nurse - Nurse Case Manager (NCM) core work hours will be from 8:00 AM to 5:00 PM, Monday through Friday, unless specified otherwise for a Medical Readiness Event (MRE). MRE's are typically held every other Wednesday and on drill weekends for Medical Detachment (MEDDET), depending on the AZARNG Operational requirements. The AZARNG works a 40-hour work week. In no case will the hours worked in any given two-week period exceed 80 hours. Qualifications Bachelor of Science in Nursing. Three (3) years of Supervisory Experience. Current, active, and unrestricted license to practice nursing in Arizona. Certification in Cardio-Pulmonary Resuscitation (CPR) for Healthcare Providers by the American Heart Association. Duties and Responsibilities Apply current medical policy, render intake case management services at Periodic Health Assessment (PHA) and Performance Requirements Summary (SRP), and identify service members' individual medical needs. Ensure that the Total Force Health Readiness Flowsheet [DD Form 2766] is complete and accurate, create Medically Non-Deployable (MND) referrals in E-Case to chart all medical correspondence. Collect all medical documentation necessary to substantiate the service member's deployment status. Ensure that all medical documentation is placed in the service member's individual medical record and scanned into their electronic record. Ensure that MEDPROS accurately reflects the service member's status, and generate provider review packets for clearing/profiling upon receipt of appropriate medical documentation. Ensure that the provider review packet is reviewed in a timely manner, and deliver the provider's disposition to the unit of assignment and the appointed medical representative Forward disposition and medical record to Health Systems Section (HSS) for board processing if applicable. Ensure all administrative charting in eCase, Dental Classification Module (DENCLASS), Medical Data Protection System (MEDPROS) and eProfile, Medical Health Assessment/Periodic Health Assessment Medical Health Assessment/Periodic Health (MHA/PHA)I s completed in a timely manner. Ensure Medical Electronic Data Care History and Readiness Tracking (MEDCHART), Automated Voucher System (AVS), Armed Forces Health Longitudinal Technology (AHLTA), Medical Operational Data System (MODS), and Medpros Web Data Entry (MWDE) are completed in a timely manner.
