The San Francisco Health Network, Population Health, Behavioral Health Services, and Administration are the core divisions of SFDPH. The San Francisco Health Network is the City’s only complete system of care and has locations throughout theCity, including Zuckerberg San Francisco General Hospital and Trauma Center, Laguna Honda Hospital and Rehabilitation Center, and over 15 primary care health centers. The Population Health Division (PHD) provides core public health services for the City and County of San Francisco: health protection, health promotion, disease and injury prevention, and disaster preparedness and response. Behavioral Health Servicesoperatesin conjunction with SFHN andprovidesa range of mental health and substance use treatment services. Job Description Under general direction, the Clinical Pharmacist performs professional pharmaceutical duties in drug education and utilization, counsels patients on drug usage, educates health care personnel on drug utilization, and may compound drugs and dispense medicines in a clinical pharmacy. The essential functions include: The essential duties of this position include but are not limited to: Interviews and examines patients for drug evaluation; checks on drug adherence and side-effects; discusses symptoms and effects of prescribed drugs with patients and maintains a drug history on patients interviewed. Consults with physicians on drug usage and the drug regime of specific patients particularly hypertensive, diabetic, and psychiatric patients; recommends changes in the drug or the dosage prescribed for these patients. Conducts both formal and informal training for nursing staff, medical and pharmacy students and residents and other health care staff on drug utilization, dosage, incompatibilities, preferred dosage forms, evidence-based medication therapy and toxicity, etc. Participates in the development and revision of medication use policies and guidelines. Compounds drugs, prepares, and dispenses medications, checks pharmacy stock, and keeps records. The 2454 Clinical Pharmacist also performs other related duties as assigned. Nature of positions: Positions are located throughout the Department of Public Health, including Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG), Laguna Honda Hospital and Rehabilitation Center (LHH), Community Primary Care and Mental Health Clinics, as well as San Francisco County Jails. Some positions may require employees to work directly with individuals within locked jails and psychiatric facilities, the homeless population and/or individuals who have or are carriers of infectious/communicable diseases. Qualifications Minimum Qualifications License:Requires possession of a valid Registered Pharmacist license issued by the California State Board of Pharmacy. (Please attach a copy of license); A. Completion of six (6) months of a pharmacy residency program; OR B. Completion of six (6) months of a post-graduate pharmacy fellowship program; OR C. Two (2) years of professional level experience as a Clinical Pharmacist (equivalent to class 2454 Clinical Pharmacist within the City and County of San Francisco); OR D. Possession of a Board Certification in a pharmacy specialty area. One year of full-time employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40-hour work week). Applicants must meet the minimum qualification requirement by the final filing date unless otherwise noted. CONDITION OF EMPLOYMENT: Appointees to positions subject to oversight by any Organized Medical Staff within the San Francisco Department of Public Health (or other applicable facilities) must maintain all required credentialed statuses, including membership, affiliated professional status, valid and current licensure, hospital privileges, and/or standardized procedures, and remain in good standing with the respective facility’s Medical Staff in accordance with bylaws, policies, rules, and regulations. Possible Special Conditions (some positions may require the following): Completion of a PGY2 Pharmacy Residency program in emergency medicine, critical care, or toxicology; OR two years of full-time professional level experience as a clinical pharmacist in an Emergency Department (ED) setting. Completion of a PGY2 Pharmacy Residency program in psychiatry; OR two years of full-time professional level experience as a clinical pharmacist in a psychiatric setting. Bilingual fluency in a variety of languages depending on the department’s needs. Only those eligibles that pass the bilingual proficiency exam will be considered for bilingual positions. The ability to obtain and maintain security clearance with the San Francisco Sheriff’s Department. Recent and verifiable experience: (1) integrating, supporting and maintaining an Electronic Health Record (EHR) or Pharmacy Information System (PIS); and (2) implementing, developing, and evaluating Computerized Prescriber Order Entry (CPOE), within the last three years. DESIRED QUALIFICATIONS: The stated desirable qualifications may be used to identify job finalists at the end of the selection process when candidates are referred for hiring. Demonstrated experience with the acute care medicine patient population. Clinical and/ or lived experience with diverse communities. Excellent oral and written communication and interpersonal skills. Ambulatory Care practice: Completion of an ambulatory/primary care pharmacy residency, specialty residency, or fellowship. Fluency in Spanish and/or Cantonese is highly desired. Mission-driven, passionate candidates with Primary Care clinical expertise and lived experience with Black/African American, Latinx, LGBTQIA+ and other diverse and intersecting communities. Meets requirements for prescribing as required by Standardize Procedure for Ambulatory Care Pharmacist within San Francisco Health Network. Behavioral Health practice: Completion of a psychiatric pharmacy residency. Clinical experience working in substance use disorder or mental health settings. Fluency in Spanish and/or Cantonese is highly desired. Possession of a valid Advanced Practice Pharmacist license in California. Completion of a geriatric pharmacy residency. Certification in Geriatric Pharmacy.
