We are partnering with a growing hospice organization serving patients throughout the Greater Springfield region. This organization is known for delivering compassionate, patient-centered care through interdisciplinary teams in both home and facility settings while maintaining a supportive, team-oriented clinical environment.

We're seeking a dedicated Hospice Registered Nurse (RN) Case Manager to join our team for a full-time position (40 hours per week).

Pay and Schedule

$85,000-$105,000 annually based on experience

• Monday-Friday schedule w/ some on-call rotation
• Flexible scheduling structure with leadership support

• Current census of approximately 100 patients
• Approximately 4-5 visits per day
• Majority of visits are facility-based, supporting more efficient scheduling

• Structured onboarding and mentorship support

Responsibilities

• Manage hospice patient caseload across home and facility settings
• Perform patient assessments and develop individualized care plans
• Provide skilled nursing care and symptom management
• Coordinate with interdisciplinary team members
• Maintain accurate and timely clinical documentation
• Maintain documentation using WellSky EMR
• Support patients and families throughout the hospice journey

T erritory

• Coverage includes Springfield, Holyoke, and Westfield
• Patients are assigned geographically to support efficiency
• Mix of facility and home-based hospice care
• Travel throughout the territory is required

Requirements

• Active RN license
• At least one year of clinical experience (acute care, home health, or hospice preferred)
• Valid driver's license and reliable transportation
