Delivering evidence-based nutrition care in hospital and clinic settings. Performing comprehensive nutrition assessments. Designing individualized care plans tailored to patients' needs. Providing counseling to patients and families managing acute and chronic conditions. Collaborating with physicians, nurses, and care teams to ensure coordinated care. Documenting care and interventions in the electronic health record (EHR). Contributing to quality improvement and population health initiatives. Role: Responsibilities Include Delivering evidence-based nutrition care in hospital and clinic settings. Performing comprehensive nutrition assessments. Strong collaboration and communication abilities. Knowledge of current evidence-based nutrition guidelines.
