Health & Wellness Director – Lead an EPIC Wellness Team! What You’ll Do * Lead & Develop Your Team: Coach, support, and grow wellness team members while creating a positive, collaborative environment. * Be Present & Involved: Stay connected with residents and staff and provide hands-on support when needed. * Create Personalized Care Plans: Develop and maintain service plans based on each resident’s needs, assessments, and physician orders. * Build Strong Relationships: Partner with residents, families, team members, and healthcare providers to ensure clear communication and coordinated care. * Oversee Resident Care: Help ensure residents receive safe, compassionate, person-centered care every day. * Manage Medications: Oversee medication management practices and promote accuracy and safety. * Assess New Residents: Evaluate prospective residents and help determine how our community can best meet their needs. * Ensure Compliance: Maintain accurate documentation and ensure wellness practices meet state regulations and Agemark standards. * Support Safety & Wellness: Identify changes or risks, respond to emergencies, and implement proactive plans to support resident well-being. * Make a Positive Impact: Help create a community where residents feel known, respected, supported, and at home. What We’re Looking For * RN or LPN: Active Nebraska nursing license. * Senior Care Experience: At least 1 year of experience working with seniors. * Leadership Experience: At least 2 years of experience leading or managing a team. * Medication Knowledge: Strong understanding of medications, dosages, and potential side effects. * Strong Leadership Skills: A compassionate, dependable leader with excellent communication and problem-solving abilities. * Tech Savvy: Comfortable using software and electronic systems. * Flexibility: Availability to support the community as needed, including occasional evenings, weekends, and Manager on Duty responsibilities.
