Case Management Director

Encompass Health seeks a Case Management Director to lead discharge planning and care coordination in our inpatient rehabilitation setting. In this role, you will oversee case managers and social workers, guide interdisciplinary team conferences, and ensure safe, timely transitions of care. You will develop processes that support regulatory compliance, utilization review, and quality outcomes while mentoring staff and partnering closely with clinical leaders.
