Schedule: Full-Time Monday to Friday Some Flexibility Offered No Evenings No Weekends No Holidays Minimal On-Call

Location: Terre Haute, Indiana

Position Description

The new physician will see patients in skilled nursing facilities and other long-term care settings. Providers work autonomously in the field and are remotely supported by a robust clinical and administrative team. A low volume of patients are seen per day, allowing ample time to focus on all needs and to provide proper attention and support. The duties of the role may include:

• Provide high-quality complex geriatric care
• Ensure engagement with facility leadership, other providers, patients, and families
• Supervise nurse practitioners and/or physician assistants
• Audit documentation for each supervised provider on a monthly basis
• Collaborate with other healthcare providers as needed
• Participate in the on-call schedule
• Attend trainings and meetings as required

The new physician will also focus on quality outcomes including, but not limited to: care path compliance, prevention of avoidable transfers, decrease in polypharmacy, costs of care. The practice uses an EHR on a mobile-based platform.

Requirements:

• Indiana medical license
• DEA
• Board Certification, relevant field (Internal / Family Medicine, Geriatrics, etc.)
• Experience supervising APPs
• 3+ years relevant clinical practice experience
