Position Summary :-

We are seeking a skilled Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) to provide high-quality, evidence-based patient care in a fast-paced outpatient clinic environment. The LPN/LVN will assess patient needs, administer medications and treatments, support clinical procedures, document patient information accurately, and collaborate with healthcare providers to promote positive patient outcomes.

Key Responsibilities:-

Administer oral, injectable, intravenous, and topical medications in accordance with established protocols and orders.

Perform routine patient assessments and monitor vital signs, including blood pressure, heart rate, respiratory rate, and temperature.

Identify and appropriately report abnormal findings or changes in patient condition.

Obtain and document comprehensive patient medical histories, including illnesses, medications, allergies, surgeries, and family history.

Conduct medication reviews and identify potential side effects, interactions, and medication-related concerns.

Perform patient screenings and assessments, including mental health screening for depression, mood, behavior, and cognitive concerns.

Provide patient education regarding medications, self-care, disease management, preventive care, and follow-up recommendations.

Perform and support phlebotomy, venipuncture, specimen collection, vaccine administration, and point-of-care testing.

Maintain accurate and timely documentation in electronic medical record (EMR) systems.

Communicate effectively with patients, families, providers, and other members of the healthcare team.

Follow all applicable safety, infection-control, privacy, and clinical protocols.

Required Qualifications:-

Full, active, and unrestricted LPN/LVN license in the applicable U.S. state, territory, commonwealth, or District of Columbia.

Minimum 1 year of related healthcare experience.

Previous experience in Primary Care, Urgent Care, or Emergency Room settings is required.

Experience with phlebotomy, vaccine administration, and point-of-care testing.

Proficiency with electronic medical record (EMR) systems; Epic experience preferred.

Strong clinical assessment, communication, documentation, organization, and patient-care skills.

Candidates whose experience is primarily limited to long-term care, nursing homes, home health, LTAC, or skilled nursing environments may not be suitable for this outpatient clinic setting.

Schedule:-

Part-time: approximately 20-30 hours per week

Typical shifts are 7–10 hours, depending on clinic operating needs.

Schedules are created in 6-week blocks following an initial 3-week training/orientation period.

Typical schedule includes two or three weekdays and every other weekend.
