Brooklyn, NY – Multiple Locations
Job Summary
Homeside Rehab is seeking compassionate and skilled Speech-Language Pathologists (SLPs) to join our growing home health team throughout Brooklyn, NY .
In this role, you will provide individualized, evidence-based speech therapy services to patients in their homes while enjoying schedule flexibility, consistent case opportunities, and support from our clinical and administrative teams.

- Brownsville – 11212
- Bushwick – 11221
- Bedford-Stuyvesant – 11216
- Brighton Beach / Sheepshead Bay – 11235
- Additional Brooklyn locations available based on patient needs
Requirements
- Active New York State Speech-Language Pathologist License – Required
- Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings preferred
- Basic computer literacy for documentation and communication
- Strong clinical and interpersonal communication skills
- Ability to work independently in a home health environment
- Reliable transportation for home visits preferred
Job Responsibilities
- Evaluate patients and develop individualized treatment plans based on patient needs and goals
- Provide evidence-based treatment for speech, language, fluency, articulation, voice, and related communication disorders
- Maintain accurate and timely documentation of patient care
- Collaborate with the clinical team to ensure coordinated and effective treatment
- Communicate effectively with office staff, patients, caregivers, and families
- Collaborate with other healthcare professionals to support comprehensive patient care
