Clinical Liaison Location: San Jose, CA. SuperCare Health is a post-acute, in-home respiratory care provider dedicated to serving patients with chronic conditions. We are seeking a Clinical Liaison to cultivate quality relationships in medical facilities, provide delivery coordination, and market our products. Essential Duties Represent SuperCare Health in multiple medical facilities, ensuring daily visibility and prompt service. Coordinate durable medical equipment (DME) and respiratory item deliveries to hospital rooms and homes alongside the Customer Care Specialist, Dispatcher, and Respiratory Department. Consult with patients and families regarding DME deliveries, usage instructions, and follow‑up care. Communicate with medical staff regarding orders, pre‑authorizations, and service coordination. Report referrals, vendor interactions, and maintain procedural compliance. Maintain inventory levels and provide billing information to the Customer Care Department. Adhere to HIPAA and company policies, submit expenses, attend meetings, and schedule community development activities. Assess patient needs and recommend appropriate respiratory therapies. Communicate with physicians about changes in patient condition, adjust plans of care, and complete related paperwork. Perform follow‑up visits for respiratory patients and support continuity of care. Serve as liaison among patients, families, caregivers, and physicians. Execute strategic planning and sales activities, including presentations to physicians and agencies. Meet monthly sales goals and quotas to drive revenue. Physical Demands Walk long distances across large healthcare facilities. Lift and carry equipment up to 75 lbs. daily. Work Environment Fast‑paced healthcare facilities requiring coordination of multiple discharges and strict adherence to deadlines. Work Experience Requirements Graduate from an accredited RT Care Program (licensed RCP). Experience as a Respiratory Therapist. Thorough knowledge and skill related to respiratory care. Excellent customer service and communication skills. People‑sensitive characteristics and genuine desire to help others. Ability to handle multiple tasks, prioritize activities, and meet deadlines.
