Location: Freeport, NY | Schedule: FT 35 hours Mon-Fri | Status: Non-Exempt |Salary Range: $24.73/hour

The Opportunity

The Case Manager provides crisis intervention, case management, family engagement, and supportive services to runaway and homeless youth and their families. Responsibilities include assessing client needs, developing and implementing individualized service plans, coordinating community resources, supporting family reunification when appropriate, and promoting youth safety, stability, and well-being. Services are provided in accordance with agency policies and applicable OCFS, FYSB, federal, state, and local requirements.

Responsibilities

• Conduct intake interviews, eligibility determinations, assessments, and safety screenings for runaway and homeless youth seeking services.

• Provide crisis intervention, supportive counseling, and family mediation services to youth and families experiencing conflict, instability, or other challenges.

• Assess client needs and develop individualized service plans in collaboration with youth, families, and service providers.

• Monitor client progress and update service plans as needed to support established goals and positive outcomes.

• Engage parents, guardians, and family members, when appropriate, to promote family stabilization and reunification.

• Assist youth with family reunification, discharge planning, transition planning, and alternative living arrangements when necessary.

• Coordinate referrals and linkages to community resources, including educational, medical, behavioral health, housing, vocational, legal, and social service supports.

• Advocate on behalf of youth and families to ensure access to appropriate services and resources.

• Provide aftercare and follow-up services in accordance with program and funding

• requirements. Maintain timely, accurate, and confidential case records, including assessments, service plans, progress notes, discharge summaries, and aftercare documentation.

• Complete required data entry and reporting in HMIS and other agency information systems. Comply with all agency policies, OCFS regulations, FYSB requirements, confidentiality standards, and mandated reporting obligations.

• Participate in case conferences, staff meetings, multidisciplinary team meetings, and program planning activities.

• Assist with transportation of youth to appointments, meetings, court appearances, and other approved activities as needed.

• Conduct community outreach, educational presentations, and tabling events to increase awareness of runaway and homeless youth services.

• Develop and maintain collaborative working relationships with community partners, service providers, schools, and government agencies.

• Participate in required trainings and maintain knowledge of trauma-informed care, positive youth development, cultural responsiveness, crisis intervention, and best practices for serving

• runaway and homeless youth. Support the daily operation of the shelter and perform other duties as assigned to ensure effective program functioning

Qualifications

• Education: Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Counseling, or a related field preferred.

• Experience:

• Minimum of one year of experience working with adolescents, families, runaway and homeless youth, child welfare, residential services, or related human service programs preferred

• Valid New York State driving license with a safe and responsible driving history.

• Active defensive driving certificate of completion. Ability to drive the agency van. Ability to obtain and maintain all required background clearances and training certifications.
