This position is in an assisted living facility in Troy, NY where an understanding of and passion for the elderly and/or mental health population is a must.Job Responsibilities Include but are not limited to:Performing admission assessments and reassessments for all residents in the homeDeveloping, monitoring, and reevaluating plans of careDay to day oversight of personal care aidesInteracting with primary care, specialty and hospital providers to ensure resident needs are being metFollowing all regulatory guidelinesTriaging and assessing residents to determine healthcare needsParticipating in continuous quality improvement program, quality assurance activities and regular chart auditsJob Qualifications:Active NYS Registered Nurse License1-3 years experience in an Assisted Living or Long Term care setting, high preferred but not requiredExperience with developing plans of care preferredExperience with various assessments such as UAS, OASIS, PRI & Screen preferred$80,000 - $100,000 Annually
