Homecare- Prefer Medicare and Oasis experience

SUMMARY
Provides nursing care is in accordance with the patient’s plan of care, including: comprehensive assessment, monitoring of the health status, coordination of services, teaching and training activities, and management of the care plan.

QUALIFICATIONS

Nursing: 1 year (Required)
Home Health Care: 1 year (Required)
RN License (Required)
Oasis Experience (Required)
Medicare Home Health (Required)

Responsibilities

Start of Care Oasis Assessments
Wound Care
Medication management
Catheter care
Infusions
Educate patients on health management and disease prevention.
Document in Brightree by MatrixCare EMR system

This position requires a background screening through the Care Provider Background Screening Clearinghouse. For information on the requirements, please visit the Clearinghouse Education and Awareness website at
