Licensed Practical Nurse / LPN - Hospice

The Licensed Practical Nurse assists in planning, coordination and care of hospice patients in accordance with the patient's plan of care. The position provides direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the interdisciplinary team to help achieve positive care outcomes. Schedule is M-F full-time 40 hours with some additional after hours/weekend back up shifts. Coverage area is Tigard, Tualatin, King City, and Wilsonville areas.

Estimated Hiring Range: $33.98 - $41.53

Essential Responsibilities

• Provide direct patient care as defined in State Nurse Practice Act following a patient assessment and an approved plan of care initiated by the registered nurse. This position may be responsible for one or more of the following areas:
• Focused/limited nursing visits
• Telephonic triage
• Night/weekend coverage back up rotation as shared among the nursing team in case the primary nurse is off
• Other duties assigned by the Hospice Leadership
• Collaborate with hospice team members regarding patient care.
• Perform focused/limited nursing assessment, recognize signs and symptoms of end-stage medical conditions/illnesses and implements planned nursing and medical interventions as delegated by registered nurse/clinician.
• Perform duties in compliance with Medicare Condition of Participation.
• Responsible for gathering and reviewing documentation in the electronic medical records, assuring accuracy, completeness and compliance with regulations, and certification standards.
• Recognize signs and symptoms of common and routine medical conditions/illnesses, abnormal versus new/existing and implements planned nursing and medical interventions as delegated by registered nurse/clinician.
• Administer medications and treatments as prescribed by patient's prescribing clinician. Evaluate patient's response to treatments/medications and notify hospice providers and/or registered nurses of concerns.
• Contribute to the development, implementation, or modification of the individualized patient's plan of care under the direction of the registered nurse/clinician.
• Provide health teaching/education and support to patients and caregivers related to end-of-life care needs.
• Assist with hospice chart auditing, as requested.

• Knowledge of Federal, State and Local regulations, Hospice Medicare Conditions of Participation, and HIPAA privacy rules and regulations

• Demonstrate nursing abilities and counseling in a hospice setting

• Must have general computer skills and a working knowledge of Electronic Medical Records systems, MS office, Outlook, and the internet

Working Conditions

Work Environment(s): Indoor/Office Community Facilities/Security Outdoor Exposure

Member/Patient Facing: No Telephonic In Person

Hazards: May include, but not limited to, physical, ergonomic, and biological hazards.

Equipment: General office equipment and/or mobile technology

Travel: Requires travel outside of the workplace at least weekly; the employee's personal vehicle may be used. Driving infractions will be monitored in accordance with organizational policy.

Work Location: Community - 5 days/week, member facing

Schedule: This position may occasionally require more than 8 hours per day and may be assigned on-call or after-hours duties.
