RN Clinical Referral Liaison - Full-Time Hospital-Based

Northwestern Memorial Hospital

Northwestern Medicine Hospital is seeking a registered nurse to serve as a liaison admissions specialist, stationed on the Northwestern Medicine Hospital campus. This 1.0 FTE exempt role focuses on clinical assessment, admission coordination, and collaboration with discharge planners and physicians.

You will assess referred patients daily, respond within 24 hours, coordinate transfers to rehabilitation, and maintain positive relationships with referral sources while ensuring smooth transitions
