LPN

We are seeking a  Licensed Practical Nurse (LPN) to help support the continuous enrichment and development of individuals with developmental disabilities in our adult residential facilities in Seymour IN.

What You Get To Do:

• Ensure that each individual receives quality individualized health care management resulting in their optimal health 
• Conducting routine site visits checking vitals doing assessments counseling of medication administration and documenting progress 
• Act as a health/medical resource for staff training staff in the medical arena
• Conduct audits and reviews of medications
• Ensure that the company is in compliance with state requirements and organizational policies regarding medical care and records 

Schedule: 

• Full-time Monday-Friday with occasional travel required;  flexible hours
• Participate in an on-call rotation for triage of medical concerns for 1 week per month

Qualifications :

What Makes You A Great Fit:

• Current  Licensed Practical Nurse (LPN) license in the State of Indiana required
• 1-2 years experience working with individuals with disabilities or other long-term care is preferred
• Must be skilled flexible self-motivated dependable and able to work cooperatively with staff interdisciplinary team members and outside agencies
• Strong verbal/written communication skills
• Ability to work independently and efficiently
• Organizational and computer skills 
• Acceptable background check & driving record check
