Job Summary:

We are seeking a compassionate and skilled Registered Nurse (RN) Concierge Nurse to join our dedicated team at Highfield Gardens Care Center of Great Neck, a sub-acute and long-term care facility in Great Neck, New York. As a RN Concierge Nurse for Weekends , you will have the opportunity to provide exceptional care and support to our residents during the weekends, ensuring they receive the highest quality care and attention.

RN Concierge Nurse Responsibilities:

• Provide direct patient care to residents, including administering medications, performing treatments, and monitoring vital signs

• Collaborate with the interdisciplinary team to develop and implement individualized care plans

• Conduct assessments, evaluations, and document patient progress

• Ensure a safe and clean environment for residents by following infection control procedures and maintaining a tidy living space

• Provide emotional support and companionship to residents, promoting a sense of community and well-being

• Participate in quality improvement initiatives to improve patient outcomes and care delivery

• Maintain accurate and detailed records of patient care and treatment plans

Requirements:

• Current RN license in the state of New York

• Prior experience in long-term care or sub-acute care preferred (new graduates welcome!)

• Strong communication and interpersonal skills

• Ability to work independently and as part of a team

• Familiarity with electronic medical records (EMRs) and ability to adapt to our system
