Job Summary

Cuready Healthcare Staffing is seeking a qualified RN Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Perform care coordination and discharge planning for assigned patients.


• Organize and participate in family meetings.


• Develop and implement individualized treatment plans.


• Coordinate with HMOs, IPAs, and other healthcare providers as necessary.


• Arrange Durable Medical Equipment (DME), Skilled Nursing Facility (SNF) placements, and ambulance transport.


• Ensure compliance with HIPAA, OSHA, and The Joint Commission standards.

Qualifications

• Active CA state RN license or eligibility to obtain upon start.


• Minimum 2 years acute care management experience, including care coordination.


• Experience with Epic charting system required.


• Proficiency in discharge planning and disease management.


• Travel experience required; first-timers not accepted.


• BLS certification and knowledge of CMS guidelines preferred.
