Position Title: Crisis Continuum of Care Case Manager Basic Function The Crisis Continuum of Care Case Manager is a member of Ada S. McKinley's Behavioral Health & Clinical Services Crisis Continuum of Care Program. This position provides comprehensive care coordination, stabilization support, case management, and linkage services to children, adolescents, adults, and families experiencing behavioral health crises. The Crisis Continuum of Care Case Manager supports clients served through Mobile Response and Stabilization Services (MRSS), the Crisis Stabilization Program, and the 590 Crisis Program by coordinating ongoing behavioral health treatment, community resources, and recovery supports. The Crisis Continuum of Care Case Manager functions as a flexible position within the Crisis Continuum of Care Program. Responsibilities may be assigned across Mobile Response and Stabilization Services (MRSS), the Crisis Stabilization Program, and the 590 Crisis Program based upon operational needs, staffing patterns, program development, and client care priorities. Principal Duties and Responsibilities Provide care coordination and stabilization services across MRSS, Crisis Stabilization Services, and the 590 Crisis Program. Conduct post-crisis follow-up and engagement to promote stabilization and reduce repeat crisis utilization. Coordinate referrals to outpatient behavioral health, psychiatry, primary care, housing, benefits, peer support, substance use treatment, and community resources. Collaborate with hospitals, schools, law enforcement, 988, 590 crisis hotline, community agencies, and other providers to ensure continuity of care. Assist with psychiatric discharge planning and transitions from crisis services. Develop individualized service plans and safety plans with clients and families. Maintain documentation in accordance with Medicaid, DHS, CARF, licensing, and agency standards. Participate in multidisciplinary staffing and case consultation meetings. Provide services using trauma-informed, recovery-oriented, culturally responsive, and person-centered practices. Knowledge and experience with Mobile Response and Stabilization Services (MRSS), Crisis Stabilization services, 988, 590 Crisis Program, behavioral health crisis systems, community-based interventions, and recovery-oriented practices. Knowledge of Medicaid behavioral health services and Illinois DHS requirements. Strong communication, organizational, and documentation skills. Ability to build collaborative relationships with clients, families, hospitals, schools, and community partners. Program Flexibility Provide services within MRSS, Crisis Stabilization Program, and the 590 Crisis Program. Maintain competency across all Crisis Continuum of Care programs. Complete all required program-specific training. Adapt to changing assignments based on operational needs. Collaborate with Team Leads, Program Manager, Director, LPHAs, Peer Support Specialists, Case Managers, Nurses, APNs, Psychiatrists, hospitals, schools, law enforcement, 988, community providers, and Managed Care Organizations. Documentation & Reporting Maintain timely and accurate electronic health record documentation. Complete documentation within agency-established timelines. Maintain compliance with Medicaid, IDHS, HFS, agency policy, and professional standards. Participate in supervision, quality improvement, and required trainings. Education and Experience Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or related behavioral health field. Must qualify as a Mental Health Professional (MHP) under Illinois DHS guidelines. Bilingual candidates strongly preferred. Experience in behavioral health case management, crisis intervention, community mental health, or hospital discharge planning preferred. Valid Illinois driver's license, reliable transportation, and proof of insurance. Compensation within the posted salary range varies based on factors including, but not limited to, experience, skills, education, and performance at the time of the offer Mental/Physical Demands: Ability to solve practical problems and deal with a variety of concrete variables in situation where only limited standardization exists. The ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel and talk or hear. The employee frequently is required to sit. The employee is occasionally required to stand and walk. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. Environmental Conditions: Work may be performed in office, school, community, residential, or home-based settings, depending on program need.May require evening hours, occasional weekend coverage, local travel, and participation in crisis related support.
