Overview
Physical Therapist, Skilled Nursing Facility in Seaford, Delaware. Travel assignment starting 12/01/2026 for 13 weeks. Weekly pay ranges from $2,439 to $2,651 per week, with guaranteed hours of 35 per week. Shift is 5x8 Days (08:00–16:30). Work setting is long-term care / nursing home. This temporary assignment is with Healthforce Healthcare LLC.

- Facility/work setting: LTC – Nursing Home Responsibilities
- Conduct initial and ongoing PT evaluations for residents in a skilled nursing facility; develop and implement individualized treatment plans.
- Provide interventions to improve mobility, gait, balance, endurance, and functional independence.
- Utilize therapeutic exercise, gait training, manual therapy, and modalities as appropriate; document progress and outcomes.
- Collaborate with an interdisciplinary team (nursing, therapy, social work, and other staff) to coordinate care and discharge planning.
- Maintain accurate, timely documentation in the resident’s health record; ensure compliance with regulatory and facility policies.
- Ensure patient safety, infection control, and adherence to best-practice standards during therapy sessions. Requirements
- Active Physical Therapist license in Delaware or eligible for licensure by endorsement.
- Ability to work in a skilled nursing facility setting with an older adult population.
- Strong communication skills and ability to collaborate with an interdisciplinary team.
- Proficiency with standard PT evaluation tools and documentation practices in common electronic health record systems. Benefits and Support
