TAY Housing Navigator & Interim Case Manager
The Village Family Services, Inc.
The Village Family Services, Inc. seeks an Interim Housing Case Manager to support Transition Age Youth (TAY) in progressing from Interim Housing to permanent housing in Los Angeles. You will assess needs, address barriers, and develop individualized service plans focused on stable housing. You will work closely with Housing Navigators, CES resources, and community partners, maintaining HMIS/CHAMP records while coordinating referrals and services to ensure timely moves to permanent housing. #J-18808-Ljbffr The Village Family Services, Inc.
