Interim Housing Case Manager

POSITION DESCRIPTION: The Interim Housing Case Manager reports to the Interim Housing Supervisor or their designee and provides comprehensive case management services to Transition Age Youth (TAY) who are enrolled in Interim Housing. The Interim Housing Case Manager works directly with participants to assess immediate needs, address barriers to housing stability, and develop individualized service and housing plans focused on achieving safe and appropriate permanent housing. The Case Manager supports participants throughout their stay in Interim Housing by providing ongoing case management, crisis intervention, housing navigation and coordination, benefits and income support, referrals, and connections to community-based supportive services. The Case Manager will work collaboratively with participants to identify and pursue appropriate housing pathways, including Problem Solving, family reunification when safe and appropriate, Time‑Limited Subsidies, Permanent Supportive Housing, and other available permanent housing resources. The Interim Housing Case Manager must have knowledge of the Youth Coordinated Entry System (YCES), HMIS, CHAMP, and TAY homeless services and be able to effectively navigate and coordinate referrals, housing resources, and supportive services. The Case Manager will collaborate with internal programs, community partners, and other homeless service providers to support participants in successfully transitioning from Interim Housing to permanent housing and achieving long‑term stability.

REQUIREMENTS & QUALIFICATIONS:

Bachelor's degree from an accredited college or university Extensive knowledge and experience in working with HSH and LAHSA programs. Minimum 2 years of experience working as a case manager with the homeless population and possess knowledge of housing and other area resources Must be proficient in record keeping, including tracking, maintaining, and reporting statistics according to program guidelines and requirements Must have the ability to meet the physical requirements of this position Experienced and strong understanding of CES/LAHSA. HSH Database experience (HMIS) strongly preferred Familiarity with TAY and HUD programs Valid California driver's license and insured automobile Knowledge and experience with motivational interviewing, evidence‑based practices, harm reduction, and trauma‑informed care Former college student or experience working in a college campus setting

DUTIES AND RESPONSIBILITIES:

All of the responsibilities listed below are considered essential functions of this position. Additionally, this position is expected to ensure adherence to the core operating values and the strategic direction of the agency. Identify and locate safe, appropriate, and sustainable housing options suitable for TAY/Young Adult households based on participants' individual needs, eligibility, and housing preferences. Provide ongoing, participant‑centered case management services to Interim Housing participants and support progress toward permanent housing and increased housing stability. Ensure participants are connected to and maintain active engagement with their assigned Housing Navigator, Time Limited Subsidies, Coordinated Entry System (CES) resources, and other internal or external service providers. Complete and regularly update individualized Housing Stability Plans, service plans, budgets, housing goals, and other required assessments or plans in accordance with contractual requirements and participant needs. Monitor participant progress toward established housing and service goals and provide appropriate interventions when barriers to housing or program participation are identified. Document case management contacts, services provided, referrals, participant progress, housing activities, and outcomes in HMIS and CHAMP case notes accurately and within required program and contractual timelines. Maintain accurate and complete participant files, CHAM & HMIS records, assessments, service documentation, and other required records in accordance with Interim Housing contractual requirements, agency policies, and applicable funding requirements. Conduct regular participant meetings and maintain required contact frequency to assess needs, review progress, address barriers, and support movement toward permanent housing. Assist participants with obtaining and maintaining identification documents, public benefits, income, employment resources, education services, health care, behavioral health services, and other mainstream benefits necessary to support housing stability. Maintain working knowledge of TAY‑specific mainstream benefits, community resources, wraparound services, housing programs, and referral pathways. Provide information, referrals, crisis intervention, problem‑solving support, and assistance with permanent housing placement to participants as part of the program's multidisciplinary case management team. Coordinate with landlords and property managers, when appropriate, to support housing placement and stabilization and provide information regarding available TAY housing programs, rental assistance programs, and community resources. Meet and communicate with prospective landlords and property managers to identify potential housing opportunities and address barriers to participant placement. Support participants with housing readiness activities, including housing applications, documentation collection, reasonable accommodation requests when applicable, lease preparation, and coordination of move‑in activities. Coordinate follow‑through services with the TVFS CES team and other housing partners and participate in care coordination, case conferencing, and housing matching meetings as assigned. Maintain knowledge of Youth Coordinated Entry System (YCES) processes and available TAY housing interventions and appropriately connect participants to eligible housing resources. Provide timely crisis intervention and coordinate with appropriate internal and external resources when participant health, safety, behavioral health, or housing stability concerns arise. Support successful transitions from Interim Housing to permanent housing by coordinating services, referrals, housing documentation, and warm handoffs to ongoing service providers. Attend weekly and monthly housing‑related community meetings, trainings, case conferences, and events as requested. Attend and, when assigned, facilitate weekly program or staff meetings. Attend regularly scheduled agency‑wide meetings, community meetings, trainings, and other meetings as assigned. Perform all responsibilities in accordance with agency guidelines, policies and procedures, contractual requirements, confidentiality standards, and applicable professional and ethical requirements. Maintain all required certifications, training courses, and professional standards applicable to the position. Arrange and maintain a work schedule that supports participant and program needs, including flexibility, when necessary, with the approval of the immediate supervisor. Perform other related duties as assigned to support the effective operation of the Interim Housing program and achievement of participant and contractual outcomes.
