Permanent Supportive Housing Case Manager

PsychPros is a nationally recognized staffing and recruiting firm dedicated to the behavioral healthcare field. We are currently hiring a Housing Case Manager to work full-time with clients experiencing homelessness in Altadena, CA. This is a TEMPORARY ASSIGNMENT with no set end date. Requirements 2 years of experience in the human services field OR a bachelor’s degree in Social Work or in a related field Possess a valid California driver's license and have access to properly registered vehicle Pay $23.50/hour Schedule Monday-Friday 8:30am-5pm Responsibilities Provide intensive case management to clients living in a permanent supportive housing program who also have a chronic health condition Conduct intake and enrollment with eligible clients Assist clients with accessing temporary housing until permanent housing is secured Support clients with the lease‑up process Conduct comprehensive bio‑psycho‑social assessments Develop individualized collaborative service plans Assist clients with their physical and mental health needs by providing support and linkage to appropriate services.
