The People Concern
The People Concern in Los Angeles is seeking a Case Manager to work onsite in the Permanent Housing program (ODR DHS) supporting clients recently released from jail as they transition to permanent housing. You will deliver intensive case management, coordinate services, and maintain up-to-date documentation. You will conduct biopsychosocial assessments, link clients to health, mental health, and recovery resources, and develop goal plans while maintaining HIPAA-compliant records. #J-18808-Ljbffr The People Concern
