Licensed Practical Nurse – Long-Term Care in New Lisbon, Wisconsin, United States. This travel, temporary assignment begins 07/06/2026 for 13 weeks. Weekly pay ranges from $1,453 to $1,562 with guaranteed hours of 36 per week. Shift is the evening PM schedule, 2:00 PM to 10:00 PM, full-time with every other weekend. Responsibilities
- Administer medications to residents following the 5 Rights and facility policy
- Observe and report changes in residents’ condition to the physician and the charge nurse
- Update family members and CNAs about changes in condition; assist with call lights and routine care
- Document care provided and communicate with the health care team; maintain accurate records
- Report allegations of abuse, neglect, or misappropriation to the Director of Nursing and Administrator
- Perform other duties as assigned by the supervising team

Requirements
- Wisconsin LPN license required; must hold and maintain licensure
- Current CPR/BLS certification required
- 1+ years of related work experience preferred
- Ability to work in a long-term care setting
- Education: LPN diploma or equivalent; or an equivalent combination of education, training, and experience
