Milwaukee Catholic Home, part of Trinity Senior Services , is a dynamic faith based community, a continuum of care retirement community on Milwaukee's east side is seeking an LPN- Wellness Nurse. The LPN - Wellness Nurse provides general nursing care and resident education within Independent Living in the Residence building as well as our Assisted Living, utilizing the nursing process, philosophy and objectives of the organization within the scope of gerontological nursing. EDUCATION, TRAINING, LICENSURE, CERTIFICATION, ETC. REQUIRED: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. Education and/or Experience: High school education and a minimum of six (6) months experience as a Licensed Practical Nurse (LPN) or RN required. Experience in a long-term care facility preferred. Must be a graduate of an accredited school of Practical Nursing and have current licensure by the State of Wisconsin, OR, Associate or Bachelor's Degree in Nursing. Knowledge and/or Skills: Must possess the understanding and ability to comply with Federal and State rules and regulations as it applies to RCAC and CBRF.
