Position Summary

The Certified Nursing Assistant / State Tested Nursing Assistant (CNA/STNA) provides personal care, assistance with activities of daily living (ADLs), and supportive home health services to clients in their private residences and community settings. Under the supervision of a Registered Nurse (RN) or Licensed Practical Nurse (LPN), the CNA/STNA promotes client dignity, independence, safety, and comfort.

This position is governed by Ohio Revised Code (ORC) Chapter 3721, Ohio Administrative Code (OAC) Chapter 3701-18 (Home Health Agency regulations), and the Ohio Board of Nursing's STNA certification requirements. All care is delivered in a manner consistent with the agency's commitment to person-centered, high-quality home health services.

Essential Duties & Responsibilities

Personal Care & Activities of Daily Living (ADLs)
  • Assist clients with bathing, grooming, oral hygiene, dressing, and toileting.
  • Provide safe and dignified assistance with transfers, ambulation, and positioning.
  • Assist with meal preparation, feeding, and ensuring adequate nutrition and hydration.
  • Support clients with light housekeeping tasks directly related to client care (e.g., client's laundry, bedroom, bathroom).
  • Assist with range-of-motion exercises and other rehabilitative activities as directed by the care plan.
  • Provide incontinence care with sensitivity and respect for client dignity.
Health Monitoring & Reporting
  • Observe and report changes in the client's physical, mental, or emotional condition to the supervising RN or care coordinator.
  • Monitor and record vital signs (blood pressure, pulse, temperature, respirations, oxygen saturation) as directed in the care plan.
  • Recognize and report signs of abuse, neglect, exploitation, or unsafe living conditions per Ohio Revised Code and agency policy.
  • Identify and report fall hazards and environmental safety concerns within the client's home.
  • Assist with medication reminders (non-administration) per agency policy and Ohio law.
Documentation & Compliance
  • Complete accurate, timely visit notes and daily activity logs using the agency's Electronic Visit Verification (EVV) system as required under Ohio Medicaid rules.
  • Document client responses to care, any observed changes in condition, and incidents or unusual occurrences.
  • Maintain client confidentiality and comply with HIPAA privacy and security regulations.
  • Complete required agency paperwork including visit logs, mileage logs, and incident reports.
  • Adhere to all Medicaid Home and Community-Based Services (HCBS) documentation requirements.
Communication & Care Coordination
  • Communicate effectively with clients, family members, and caregivers regarding the care plan and any concerns.
  • Maintain a professional, respectful, and compassionate relationship with clients and their households.
  • Participate in care plan conferences and interdisciplinary team communications as needed.
  • Notify the agency promptly of schedule conflicts, client cancellations, or inability to report to an assignment.
  • Collaborate with the RN supervisor to ensure continuity and consistency of care.
Safety & Infection Control
  • Follow standard precautions and infection control protocols including proper hand hygiene and use of PPE.
  • Maintain a clean and safe environment for the client during all visits.
  • Respond appropriately to client emergencies, including calling 911 and notifying the supervising nurse.
  • Complete and maintain current CPR/First Aid certification.
  • Comply with OSHA bloodborne pathogen standards and agency safety policies.
Professionalism & Agency Standards
  • Arrive on time to all client assignments and maintain reliable attendance.
  • Present a professional appearance and adhere to agency dress code.
  • Represent the agency positively in all client homes and community interactions.
  • Complete all required agency training, annual in-services, and competency evaluations.
  • Comply with all agency policies, Ohio home health regulations, and Medicaid provider standards.
Qualifications

Required
  • Current, valid STNA certification listed on the Ohio Nurse Aide Registry (OBN) in good standing with no findings of abuse, neglect, or misappropriation.
  • Minimum six (6) months of CNA/STNA experience; home health or community-based experience preferred.
  • Current CPR/First Aid certification (American Heart Association or equivalent).
  • Valid Ohio driver's license, reliable personal vehicle, and current automobile insurance.
  • Successful completion of criminal background check per ORC 3721.121 (Ohio Nurse Aide Registry background screening).
  • Ability to pass a pre-employment drug screening.
  • Ability to read, write, and communicate effectively in English.
  • Basic computer or smartphone literacy for Electronic Visit Verification (EVV) compliance.
Preferred
  • One (1) or more years of home health aide or personal care experience.
  • Experience with elderly clients, individuals with disabilities, or those with chronic illness.
  • Familiarity with Ohio Medicaid HCBS waiver programs (e.g., PASSPORT, Assisted Living, Ohio Home Care waiver).
  • Bilingual or multilingual skills are a plus.
  • Reliable transportation with willingness to travel within assigned service territory.
Compensation & Benefits

Pay Rate:

$16.00 - $19.50 per hour (based on experience & availability)

Pay Schedule:

Weekly / Bi-Weekly (per agency policy)

Mileage:

Reimbursed per agency policy for travel between client visits

Overtime:

Paid at 1.5x for hours worked over 40 per week (FLSA)

Benefits:

May include: PTO, health insurance, dental/vision (eligibility varies by FT/PT status)

Training:

Paid orientation, annual in-services, and skills competency training

Advancement:

Physical Requirements

The following physical demands are representative of what is required to perform essential job functions:
  • Ability to lift, push, pull, and carry up to 50 lbs.; assist with transferring clients with or without mechanical lifts.
  • Ability to stand, walk, bend, kneel, stoop, and reach for extended periods throughout the workday.
  • Sufficient visual and auditory ability to monitor client conditions and communicate effectively.
  • Ability to drive personal vehicle between client assignments within the service territory.
  • Ability to respond quickly and appropriately in medical emergency situations.
Work Environment
  • Services are performed in private residences, assisted living facilities, and community settings throughout the agency's service area.
  • Work environment varies by client home; the agency does not control home conditions.
  • Exposure to potential hazards including bloodborne pathogens, infectious diseases, and physically dependent clients; training and PPE are provided.
  • Schedule may include mornings, evenings, weekends, and holidays depending on client and agency needs.
  • Travel between multiple client assignments in a single workday may be required.
