RN Case Manager - Patient Care & Care Coordination
BAKERSFIELD FAMILY MEDICAL GROUP, INC.
Bakersfield Family Medical Group, Inc. is seeking a Registered Nurse Case Manager to join the Priority Care Program in Bakersfield, CA. Under the direction of the Case Management Supervisor, you will assess, plan, implement, and evaluate patient care, coordinating with providers to optimize outcomes and reduce ER utilization. You will conduct annual comprehensive assessments, develop SMART goals, document in NextGen, and educate patients and families about benefits, resources, and care options #J-18808-Ljbffr
