What You Should Know About the LPN Extended Hours - Individualized Care Role:

• This is a Full-Time DRIVING position covering Wooster and surrounding areas
• Hours are 4P-12A 3 days a week and 8a-8p every other weekend. Also include holiday requirements.
• We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed

The LPN Individualized Care Essential Duties Are:

We provide superior care for patients in families in homes/facilities. The RN will provide quality nursing care to patients in a manner that meets the patient and family needs, working under the direction and supervision of an RN. You will provide superior care to patients and their families.

LPN Individualized Care Qualifications:

• Registered LPN with the State of Ohio without any board actions.
• One year of nursing experience preferred.
• CPR Certified.
• Computer skills sufficient to properly document services and care.
• Ability to drive during daytime, nighttime or inclement weather.
• Valid Driver's License with Safe Driving Record.
• State Minimum Automobile Insurance Coverage.
• Must be able to pass a criminal background check
• Must be able to pass a 10 screen drug panel
