Case Manager - Full Time

Department: NURSE CASE MANAGERS – 208415 • Worker Sub Type: Regular Work Shift • Pay Range: $34.00 – $48.87 Position Overview The Case Manager collaborates with an interdisciplinary team to develop and document a discharge plan, advocates for patients and families, facilitates, brokers, and coordinates plan implementation, and continually modifies the plan based on patient needs. Duties and Responsibilities Negotiate services and resources when deemed needed, employing creative problem‑solving for complex discharge planning. Participate actively in discharge‑planning rounds and process‑improvement projects. Communicate patient needs and updates to appropriate professionals and post‑acute providers. Collaborate to ensure continued acute hospitalization remains appropriate. Maintain supportive, dynamic relationships with all multidisciplinary team members and post‑acute providers. Complete thorough, timely documentation that accurately reflects the discharge plan status. Screen assigned patients within 24 hours or the first business day. Serve as a resource and educate the multidisciplinary team on levels of care, transition‑planning issues, and regulatory concerns. Stay informed of research and current practice trends via professional meetings, in‑services, and conferences. Maintain knowledge of reimbursement modalities, community resources, and legal issues affecting patients and providers. Assist with arrangement and follow‑up with community resources. Ensure a seamless transition across the continuum of care, facilitating patients’ movement to the next level of care. Education and Licensure Bachelor’s degree in Nursing (BSN) – required. Master’s degree in Nursing (MSN) – preferred. Graduate of an accredited school of nursing – required. Experience Minimum five years of professional practice experience in an acute‑care setting – preferred. Prior Case Management or Utilization Management experience – preferred. Certification Licensed Registered Nurse in the State of Virginia – required. BLS Certification (Basic Life Support) – required. American Heart ‘Healthcare Provider’ (HCP) – AHA approved – required. New hires must possess AHA‑approved certification prior to orientation. Certification in Case Management – desirable. Qualifications Excellent written and verbal communication and interpersonal skills with all customer levels. Critical‑thinking, creative problem‑solving, and proficient organization and planning. Self‑directed, tolerant of frequent interruptions, and able to manage a demanding workload. Knowledge of funding, resources, services, clinical standards, and outcomes – preferred.
