Practices the pharmacists' patient care process in providing safe and effective drug therapy. Provides clinical and legal review of all new patient prescriptions. Maintains pharmacy records in accordance with applicable laws and regulations. Collects and assesses relevant information to develop patient focused plan including medication appropriateness, medication reconciliation, and monitoring parameters including laboratory and clinical data. Provides recommendations to prescriber for plan alterations when appropriate to enhance outcomes associated with drug therapy. Provides medication consultations to patients and dispenses all medications and related supplies and equipment in accordance with federal and state laws and standards of practice which govern the profession of pharmacy and sterile compounding. Checks all medications, supplies, and equipment prior to dispensing from the pharmacy. Completes thorough and timely documentation of patient notes and charting in the organization's electronic health record. Manages patient care as delegated by a physician through any written delegation agreements. Maintains clinical skills and knowledge of pharmacotherapy by reviewing the literature and attending in-services, seminars, and/or conferences. Successfully completes initial and ongoing organizational and department education, training, and competency requirements. Infection Control: Understands, demonstrates, and follows organizational infection control guidelines (CDIC). Quality: Focuses on improving processes and champions efficiency in daily work activities. Safety: Promotes a culture of safety by taking accountability for individual actions, including but not limited to, promptly reporting errors or near misses via the organization's occurrence reporting system and participating in developing improved safety processes, as appropriate. Performs other duties as assigned.Job RequirementsLicenses: - State of Wisconsin Professional licensure for PharmacistEducation: - Bachelor's degree in: PharmacyPharmDExperience: - Completion of a PGY-1 hospital residency or minimum of 2 years of recent experience in an acute care setting or one year of recent experience in home infusion therapy preferred. Experience and strong knowledge base in intravenous admixture services and intravenous drug therapy preferred.Miscellaneous: - Individuals who are pharmacist vaccinators are required to be CPR certified. Board certification in nutrition support required within the first year of employment.
