Position Summary: This position is responsible for accepting referrals for admissions into the TCM and CSP programs, conducting the intake interviews, completing intake paperwork, and potentially carrying a small caseload until assigned to a permanent case manager. Essential Functions: Accept referrals for admission into the TCM/CSP programs; enter all referral information in the computer, review all medical records, and schedule intake appointments for all clients Open TCM/CSP episodes in Provider Connect Conduct intake interviews Place the medical records in appropriate file folders Inform Benefits staff of the new client, what benefits need to be applied for, and input billing information Update spreadsheet to track all new admissions, civil commitments, and sex offender clients Arrange for client transportation to and from WCS at the time of intake Assist with assigning case managers to new clients based on caseload numbers Other Duties and Responsibilities: Attend staff meetings as needed Assist with case management duties when not fully staffed as time allows Job Qualifications: Minimum Qualifications : Master’s Degree in a Behavioral Science with documented 1500 hours of experience working with persons with serious and persistent mental illness No convictions or license revocations that would fail a caregiver background check Valid driver’s license with automobile and sufficient insurance to meet agency requirements Knowledge, Skills and Abilities: Good computer skills including Word and Excel; accurate data entry; good organizational skills and the ability to multitask; ability to meet deadlines; some knowledge of mental illness and the ability to work with individuals diagnosed with mental illness. pm21 Job Responsibilities: Position Summary: This position is responsible for accepting referrals for admissions into the TCM and CSP programs, conducting the intake interviews, completing intake paperwork, and potentially carrying a small caseload until assigned to a permanent case manager.
