Program Coordinator

Seeking a Program Coordinator to support day services (Day Hab) for individuals with intellectual and developmental disabilities (I/DD). The role focuses on Life Plan/person-centered planning, documentation, OPWDD and Medicaid compliance, quality assurance, admissions, and interdisciplinary coordination. Responsibilities: CoordinateLife Plans, Staff Action Plans, and person-centered services Review and maintainprogram documentation for accuracy, completeness, and compliance MonitorPrecisionCare/EHR records and ensure documentation supports Medicaid billing Conductquality assurance audits and track corrective actions Ensure compliance withOPWDD, Medicaid, HIPAA, and agency requirements Coordinateadmissions, transfers, and discharges Collaborate withCare Managers, nursing, clinical staff, families, and interdisciplinary teams Participate inLife Plan, clinical, medical, and team meetings Supportaudits, surveys, and corrective action plans Identify documentation/workflow issues and recommendprocess improvements Promoteperson-centered practices and ensure services align with individuals’ goals, preferences, and outcomes Requirements: Bachelor’s degree in Human Services, Social Work, Psychology, Healthcare Administration, or related field 2+ years of experience working with individuals with I/DD Experience withOPWDD regulations and documentation Knowledge of person-centered planning and Medicaid requirements Strong organizational, communication, and documentation skills PrecisionCare/EHR and Life Plan experience preferred Hours: Monday through Friday 8:00AM to 3:00PM.
