Position Details Setting : Academic Internal Medicine / Value-Based Care Clinic Patient Population : Adult / Complex Internal Medicine Schedule : Full-Time Permanent Day Shift Hours : Full-time daytime clinic-based schedule; standard business/daytime hours Location : Dallas, TX Compensation : Competitive salary based on experience and training candidates may submit salary expectations with presentation Start Date : Open This is a dual-mission position combining academic faculty responsibilities with direct patient care in a value-based care clinic. You will serve as core faculty for the Internal Medicine Health Equity Track, supervise residents in a collaborative academic setting, and deliver high-quality, population-centered care addressing the social determinants of health. The position is affiliated with both the health system and a prominent academic medical university. Teaching & Educational Leadership Serve as core faculty for the Internal Medicine Health Equity Track Supervise Internal Medicine residents in a clinic-based experiential learning environment Provide real-time clinical teaching in primary care, population health, value-based care, and health equity-focused practice Teach residents to integrate social determinants of health, community-engaged care, and equity-centered clinical decision-making Participate in journal clubs, lecture series, case conferences, and resident workshops Mentor residents in quality improvement initiatives, health equity research, and scholarly activities Collaborate with residency and program leadership to develop curriculum aligned with ACGME competencies Provide formative and summative feedback to residents to support professional growth Direct Patient Care Responsibilities Deliver comprehensive Internal Medicine care within the Value-Based Care Clinic Utilize evidence-based guidelines and value-based care principles Help establish and refine value-based standard practices for the clinic Use population health tools, risk stratification, and care coordination strategies Improve patient outcomes and reduce healthcare disparities Identify and address non-medical drivers of health impacting patient care Deliver coordinated, team-based care alongside care managers, pharmacists, social workers, behavioral health specialists, and community partners Partner collaboratively with Graduate Medical Education personnel ACGME Core Competency Alignment Provide compassionate, effective care for complex adult patients Model equitable and high-value care for residents Maintain current expertise in Internal Medicine, health equity science, and value-based care frameworks Integrate evidence-based medicine into both patient care and teaching Participate in quality improvement projects and mentor residents in QI methodology Support operational plans, coordinated care delivery, and clinical process improvement Demonstrate advanced communication, leadership, innovation, and problem-solving skills Location : Dallas, TX. Minimum Specifications Education Must have successfully completed an Accreditation Council for Graduate Medical Education (ACGME) or American Medical Association (AMA) approved residency for appropriate specialty. Experience None Certification/Registration/Licensure Must be currently licensed for the practice of medicine in the State of Texas. Must meet and maintain licensing and registration requirements as delineated by the Medical Staff Bylaws, Medical Staff Rules and Regulations, and Credentialing Policies/Procedures, if any. Skills or Special Abilities Must be able to demonstrate a working knowledge of the appropriate specialty practice (Family Practice, Internal Medicine) assessment techniques and treatment processes. Must be able to determine objectives, set priorities, and establish and meet target dates. Must be able to work effectively with other health care professionals and serve as a resource to the community. Must be able to demonstrate patient centered/patient valued behavior. Responsibilities \ 1. Patient Care\ a. Gathers and documents essential and accurate information about patients; makes informed decisions about diagnostic and therapeutic interventions incorporating patient information and preferences; develops and carries out patient management plans on an ongoing basis.\ b. Provides health care, in‑person and/or virtual, and utilizes evidence-based clinical practice guidelines in the management to improve health outcomes. Virtual care is provided through the use of audio/video patient encounters, or by audio-only patient encounters.\ c. Counsels and educates patients and their families regarding health promotion, appropriate management of acute conditions, and self‑management of chronic conditions.\ d. Collaborates with other health care professionals within and outside of PHHS, including community resources when appropriate, to provide coordinated patient‑centered care. Interpersonal and Communication Skills\ e. Utilizes effective interpersonal and communication skills with patients, families and the care team. Creates and maintains a therapeutic and ethically sound relationship with the patient. Demonstrates respect, compassion, integrity and responsiveness to the needs of the patient.\ \ 2. Practice‑Based Learning and Improvement\ a. Maintains knowledge and works toward the division quality goals to achieve the desired practice outcomes.\ b.
