RN Case Manager

Job Description The Case Manager is typically a professional ensuring that patient care is coordinated and managed appropriately. Case Management is in charge of staffing and plotting visits for SN (skilled nurses), MSW, CHHA, calling patients after SOC/RECERT/ROC, assigning supervisory visits for LVNs and CHHAs, ensuring timely submission of the Oasis/visit notes, coordinating lab/pharmacy/meds/staff/MD/track/obtain authorization for HMO, coordinating DME orders and supplies, ensuring a CHHA plan is present if applicable, informing visiting staff when patient is transferred to Hosp/SNF and after ROC. Requirements One (1) year experience in home health and/or case management. Must be able to pass a background check. Eligible to work in the US. Basic Microsoft Office Excel, Word, and Anti‑Virus knowledge. Strong attention to detail and note‑taking ability. Strong organizational skills. Positive attitude and team success mindset. Willingness to study, train, and learn new skills. On‑call company cell phone for after‑hours (rotational).
