Emergency Department Case Manager (RN) - Night Shift

Job Description Emergency Department Case Manager (RN) needed for a night-shift contract opportunity with Yoh’s client located in Palo Alto, CA. Responsibilities Responsible for coordinating continuum of care and discharge planning activities for a caseload of assigned patients. Coordinating all facets of a patient's admission/discharge. Performing utilization review activities, including review of patient charts for timeliness of services and appropriate utilization of services. Ensuring optimum use of resources, service delivery, and compliance with external review agencies’ requirements. Act as consultant to the clinical team, service lines, and other departments regarding patient assessment and patient care, and participate in program development and quality improvement initiatives. Apply guidelines and collaborate with multidisciplinary teams, influence and direct the delivery and quality of patient care. Qualifications Active and unrestricted California RN license. Bachelor’s Degree in Nursing preferred. Current BLS certification. Two (2) years of progressively responsible and directly-related work experience. Previous case management experience, preferably in an acute care and emergency department nursing experience setting. Excellent verbal and written communication skills. Excellent computer skills.
