RN Case Manager

Education Level4 Year Degree**Physical Demands:** Duties are generally performed in an office environment where hazards and discomforts are controlled and modifiable. This position requires the ability to stand, bend at the waist, kneel, reach over the head, talk, hear, and see. Must be able to move or lift documents and materials weighing up to 30 pounds. Position requires knowledge and use of typical office equipment including telephones, copiers, fax machines, and personal computers. Position requires frequent contact with fellow employees, patients and community. Position may require travel to other sites for work assignments, meetings or training.## Qualifications**Qualifications - Education/Experience:*** Ability to develop effective interpersonal relationships with health providers and management.* Ability to perform duties across life cycle (infant, children, adolescents, adults, prenatal and geriatric).* Driver's license and clean driving record is required.* Demonstrated organization skills, time management and ability to manage multiple cases.* Strong communication skills, writing and verbal. Bilingual in English and Spanish strongly preferred.* Knowledge and experience in HIPAA regulations, information privacy/security laws, and access and release of information.* Knowledge of "infection control protocols" and experience in teaching/educating patients about care of chronic diseases.* Computer proficiency.**Required Vaccines and Test:** Influenza, Tdap, Hep B, MMR, Varicella, TB test, and Negative Drug Test results.
