Pay: $18-$20/hr.

Patient Care Coordinator Duties

Coordinate patient admissions for home infusion and infusion suite services.
Serve as the primary liaison between patients, physicians, nurses, pharmacists, and other healthcare professionals.
Obtain, review, and process physician orders, referrals, and supporting medical documentation.
Verify insurance benefits and assist with prior authorizations and coverage requirements.
Schedule patient appointments, infusion services, and supply deliveries.
Maintain accurate patient records and ensure documentation meets regulatory and company standards.
Educate patients and caregivers about services, treatment plans, and care processes.
Monitor ongoing patient needs and communicate updates to the clinical team.
Address patient questions, concerns, and service issues while providing excellent customer service.
Coordinate start-of-care activities to ensure timely initiation of therapy.
Work closely with intake, pharmacy, nursing, and billing departments to support continuity of care.
Assist with insurance, billing, and reimbursement-related communication when needed.
