NPORT Staffing is seeking qualified Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to join our Home Care Team.

**Physical Therapist (PT) – OASIS Evaluations**

We are seeking experienced PTs to provide home care OASIS evaluations and assessments for a summer coverage assignment.

**Position Details: **

* Summer coverage assignment

* Up to 13-week contract

* Flexible scheduling

* Competitive compensation

* Home care evaluations and OASIS assessments **Physical Therapist (PT) – Home Care**

**Requirements: **

* Active New York State PT license

* OASIS certification required

* Home care experience preferred

* Reliable transportation

* Strong assessment and documentation skills

**Physical Therapist Assistant (PTA) – Home Care**

**Position Details: **

* Ongoing position

* Flexible schedule

* Consistent patient visits

* Immediate start available

* Pay rate: $53.60–$56.35 per visit

**Requirements: **

* Active New York State PTA license

* Home care experience preferred

* Reliable transportation **Qualifications**

* Excellent communication and patient care skills

* Ability to work independently
